                                    ?          Social Security cards for 
you, your spouse, and all

                                    ?          Birth dates for you, your 
spouse, and all dependents on the

                                    ?          Childcare provider 
information

                                    ?          Interest, dividend, tuition, 
and mortgage forms 1099 and 1098

                                    ?          Bank routing number and 
account number for direct deposit



Your returns are completed by IRS Certified Volunteers.  Returns are filed 
electronically.  To file electronically on a married-filing-joint return, 
both spouses must be present to sign the form. Information is kept 
confidential.  This service is sponsored by RSVP of Durham County, the IRS, 
Durham Technical Community College, Friends of Durham County RSVP, and
