We are having similar issues, even on the same remit.  Some claims are paid and some denied.  Some they have even put to "patient responsibility" for certain CPT codes.  We cannot figure out any common denominator for the denials.

I AM USING MODIFIER QW

I CURRENTLY HAVE ABOUT 1/3 OF MY CLAIMS PAID AND THE OTHER 2/3 BEING REJECTED FOR NON COVERED SERVICE.

ARE OTHER OFFICES HAVING THE SAME PROBLEM?
