Safety flash information shared yesterday with BP from the fatal
contractor incident investigation involving a man lift. 

After the fatal accident involving a man lift that occurred at BP's
Texas City site July 21, an investigation was conducted by off-site
experts. The following is a high-level summary of the complete accident
investigation report. In addition to an account of the incident, it
includes corrective actions and lessons learned. 

In brief, equipment needs to be in proper working order for safe use and
only used for its intended design purpose.

Please share the learning with your work teams and emphasize equipment
design, maintenance and proper use practices. 

Share as appropriate.
