[Durham INC] Community survey- ends tomorrow!

Hi,
At the health department, we've been working on a community health assessment for a little over a year now and are close to completing it. We conducted a random sample of households in the fall of 2016 and used survey responses and input from other community members to create a list of 11 potential priority areas for Durham County. We would like to narrow that list down to five. After five are selected, we form committees around each topic area and create action plans to address the issues. We meet monthly with community members and community organizations to continue the work through the Partnership for a Healthy Durham.
