NCCARE360 has had so many exciting changes and updates over the past six
months and I wanted to give our community partners the opportunity to
attend this upcoming event to learn more about what's been happening with
NCCARE360. I hope you'll consider attending! If you're not able to make it

*About: *Join us for an exciting Impact Review where you will have the
opportunity to learn more about the progression and impact of NCCARE360 in
your area! During this virtual session, you will hear from NCCARE360
administrators and partner organizations, see data relevant to trends in
your community, hear about exciting new platform offerings, and have a
chance to connect with other health and social care providers in your
community. We will also take some time to go over the NCCARE360 Health
Equity Grants which were awarded to several area providers.
