Subject: HELP NEEDED! Durham Community Health Assessment Survey Starting September 21st 

 

Good afternoon everyone! 

 

I hope you are well. I am writing to announce that the Community Health Assessment (CHA) survey starts Wednesday, September 21st. Surveys will be administered Wednesdays-Saturdays 10am-6pm each week until 630 surveys are completed. The CHA survey is administered every three years by Durham County Department of Public Health, Duke Healthy, and Partnership for a Healthy Durham. This is a very important assessment. The data collected shapes health programs and funding. 

 

We need volunteers to go door-to-door to learn more about the community’s health concerns and priorities. Volunteers will receive training, t-shirts, and lunch!

              If you are unable to attend a training, there will be virtual training available. Also, if you have administered CHA surveys before, there is an abbreviated training available the day of your shift.
