<p class="x_MsoNormal">I hope you are well. I am writing to announce that the Community Health Assessment (CHA) survey starts Wednesday, September 21<sup>st</sup>. Surveys will be administered Wednesdays-Saturdays 10am-6pm each week until 630 surveys are completed.
The CHA survey is administered every three years by Durham County Department of Public Health, Duke Healthy, and Partnership for a Healthy Durham. This is a very important assessment. The data collected shapes health programs and funding.

<p class="x_MsoNormal">We need volunteers to go door-to-door to learn more about the community’s health concerns and priorities. Volunteers will receive training, t-shirts, and lunch!
