[Ohioaap-practicemanagers] payment for being a medical home

Hi all,
   The American Academy of Pediatrics has some questions that would be worthwhile for us to chime in on. They're on a subject for which we can help each other.
Here goes:
 1) Is any health plan, private or Medicaid, offering you payments or other incentives to meet their criteria for being a "patient centered medical home"?
 2) How are they defining "medical home"? Some places want a practice to be NCQA certified, but some plans see that as being hard for practices and might have a more practical definition
 3) Were you able to earn the incentives? Any hints for your colleagues on how most efficiently to do it?
