Subject: Ohio Comprehensive Primary Care (CPC) Webinar - 2019 Program Updates and Enrollment

Good afternoon,

On Thursday, August 30 at 11:30am, the Ohio Department of Medicaid will host a webinar about the Ohio Comprehensive Primary Care (Ohio CPC) program. This webinar will include general information about Ohio CPC, as well as details about changes to the program for 2019 and information about upcoming enrollment.

Ohio CPC is Ohio Medicaid’s patient-centered medical home (PCMH) program: a team-based care delivery model led by a primary care practice that comprehensively manages a patient’s health needs. The aim is to empower practices to deliver the best care possible to their patients, both improving quality of care and lowering costs.

Currently, 145 practices participate in Ohio CPC, covering over 1 million Medicaid lives. 2019 will be the third year of the program. We encourage you to share this invitation with other practices that may be interested in learning more about Ohio CPC.
