Durham County Department of Public Health is collecting data to provide COVID-19 vaccine information and resources to Durham County residents in the areas they are needed most. This is a brief, anonymous survey that can be done in about 3 minutes. They would like to get responses from a representative group as possible. Please share this with your networks.
