Dear Colleagues,

There have been 204 million confirmed COVID-19 cases and 4.13 confirmed
COVID-19 deaths around the world.  The actual COVID-19 infections and
deaths are most likely many times these confirmed numbers.  Unfortunately,
the vast majority of the human population worldwide do not have acquired
immunity to protect them against the rapidly mutating SARS-CoV2 virus.
Additionally, with breakthrough SARS CoV-2 variants, we do not know how
long our current vaccines will keep us safe from severe COVID-19.

The U.S., which has prided itself on having some of the most advanced
health systems and biosecurity capacities, is still the most COVID-19
impacted society in the world in terms of confirmed cases,
hospitalizations, and deaths.  What is this saying about American society
at this time?  Even as one of the societies with the most vaccinated
populations, the U.S. is also one of the societies most impacted since July
4 in terms of a sweeping upsurge of Delta variant COVID-19 cases.

A dispassionate, politically-independent view of the spread of the Delta
variant leads one to think that some states with high vaccination rates,
use of masks, good social distancing, and low confirmed cases with few
COVID hospitalizations and deaths must be doing something right.  Those
states and communities with low vaccine-acquired immunity, as well as
cultures and leaders standing against masks, poor social distancing, and
exponentially increasing COVID hospitalizations and deaths must be doing
something seriously wrong.  Given that the American public has been
provided free vaccines, effective masks, and the information that enables
them to understand the safety and efficacy of the vaccines, mask use, and
social distancing, where are the key solutions that can overcome this key
conundrum for those individuals and communities at risk?

Please join us to talk about the need for an essential change in pandemic
strategy for the remainder of 2021 and into 2022, reflecting on what has
happened in the past year with high COVID cases and deaths in late 2020 and
early 2021.  Our hypothesis is that a new bottom-up, community-centric
strategy is now essential to improve pandemic and syndemic resilience
across the United States during Quarter 3 and 4 of 2021 and beyond.  The
Resilient American Communities (RAC) unity of effort is working with the
Alliance for Prevention and Response and Geiger-Gibson 2.0 to integrate
this insight and our collective civil society-driven efforts with the
efforts of HHS, the White House, Congress, the National Governors
Association, and the National League of Cities to reduce the impact of the
Delta variant and other emerging variants through hyper-local approaches to
vaccination equity and behavioral and social resistance to COVID even in
the hardest to reach American communities striving to achieve better
health, resilience, and regeneration.

The morning will begin with updates on the COVID-19 pandemic and syndemic
trends and progress on the Resilient American Communities COVID-19
initiatives.  Special attention will be paid to the dynamics that have
driven significant increases in COVID infections, hospitalizations, and
deaths since July 4.  We will look at the COVID numbers across states and
communities considering school season start, Delta variant immune escape,
the impact of other variants, CDC's reimplemented mask recommendations in
internal spaces even for those with two doses of COVID-19 vaccine.

Points of Discussion

What are the key factors in reducing cases, hospitalizations, and death in
this phase of the COVID-19 pandemic, with vaccinations not increasing at a
sufficient rate to achieve acceptable levels of herd immunity in many
states and communities?
Why are the southern, midwestern, and western states (such as Florida,
Louisiana, and Texas) being so inordinately impacted?

Why has Region 1 (New England) and the Northeastern communities generally
done far better with vaccinations and infection control than many states
and communities in the Mid-west, West and the South that are now at high
risk of COVID infections?

Are the vaccination rates changing dramatically in hardest-hit states and
communities?  If not, why are they so slow to adapt to increasing risks?

How will the Delta, Gamma, and Epsilon variants affect infection rates,
pandemic conditions, and potentially trends in immune escape?

Is it possible to we could see an emerging COVID-19 variant with both
immune escape and high lethality in the vaccinated populations or in
younger populations?

How will RAC efforts enhance vaccination equity and health equity in the
most vulnerable and disenfranchised populations with micro-targeted data
and hyper-local community engagement?

What kind of grants, contracts, and new forms of unity of effort are making
a key difference in resilient American communities?

Informal discussions of the increasing impact of COVID-19 variants and
vaccine-acquired immunity escape, as well as new interests in the origins
of the COVID-19 pandemic in Wuhan, China and climate change preparations in
Resilient American Communities.

Janice Lucas, from the RAC Bay County will facilitate the afternoon updates
and discussions.

Special Presentation:  Dr. Lisa Merritt will keynote this session with
discoveries of engaged community solutions within RAC-related MHI efforts
in Sarasota and Manatee counties.

The status of RAC sites around the U.S. will be reviewed in regards to
stages of development and use of RAC systems

Regions, states, and communities will provide updates on the progress of
their work.



*3:00 PM - 3:30 PM Break and Networking*

Ed Shanshala, chair of the RAC Youth Center of Excellence, will facilitate
further discussions regarding outbreaks in younger populations  and the
challenges surrounding the start of the 2021 school season with
intergenerational impacts.

*3:30 PM - 4:30** PM **Presentation and Reflections *
Ed Zuroweste will facilitate a discussion about RAC’s approach to the
Geiger - Gibson 2.0 (G-G 2.0) initiative outside the four walls of the
FQHCs.  Concepts will be explored for the integration of G-G 2.0 fellows
and collaboratives aligning with CBO and public health initiatives within
the Resilient American Communities (RAC) unity of effort.  What could this
mean for improving vaccination equity using landscape analyses and
micro-targeting.

Points of Discussion

What would a Geiger / Gibson 2.0 movement look like with RAC interacting
with FQHCs and the APHA Alliance around the U.S.?

What would a RAC Landscape Analysis initiative look like focused on
vaccination and health equity rapid responses to resolve gaps during the
upcoming stages of the COVID-19 pandemic and syndemic?

What kind of RAC upstream hyper-local data would provide micro-targeting
benefits of vaccinations and other capacities and resources to America’s
most vulnerable communities?

How would thousands of RAC sites interact with thousands of FQHC clinical
sites and their community outreach efforts?

How would RAC Centers of Excellence potentially interact with FQHCs and
Public Health in HHS regions, states and counties?

The national COVID risk maps above depict where the impact of the pandemic
will likely affect health, human security and syndemic factors during the
upcoming weeks at the state and county levels given the risk levels
measured this week.  In addition to increasing vaccinations and social
equity, it is again time to be proactive about behavioral and social
resistance to COVID-19 infections, transmissions, and translocations
amongst those at greatest risk in the hyper-local levels where COVID cases
are sky-rocketing and where new outbreaks are just emerging.  These
achievements will happen most readily now at the hyper-local through trust
relations, such as family and friends networks and broader coalitions of
the willing.
